Source: Coumestrol is a naturally occurring coumestan found in legumes and other plants, including alfalfa, clover, soybeans, spinach, Brussels sprouts, and Pueraria lobata.

Function: Coumestrol is a phytoestrogen that binds estrogen receptors ERα and ERβ, with substantial estrogen-receptor activity. It also exhibits antioxidant, anti-inflammatory, antiproliferative, and neuroprotective effects and can modulate PI3K/AKT, MAPK, HIF-related signaling, mitochondrial function, and cholinergic enzymes.

Cancer: Coumestrol generally demonstrates anticancer activity in preclinical models, including proliferation ↓, migration/invasion ↓, mitochondrial membrane potential ↓, ROS ↑, apoptosis ↑, caspase activation ↑, and modulation of PI3K/AKT, ERK, JNK, and HIF signaling. Because coumestrol is a potent phytoestrogen and activates both ERα and ERβ, its effects in hormone-sensitive cancers can be concentration-, receptor-, and tissue-dependent.

Alzheimer's Disease: Coumestrol has demonstrated experimental neuroprotective activity relevant to Alzheimer's disease. Reported mechanisms include antioxidant and peroxynitrite-scavenging activity and inhibition of acetylcholinesterase and butyrylcholinesterase, which may support cholinergic signaling. Broader evidence also supports anti-inflammatory and neuroprotective effects, although clinical evidence for Alzheimer's disease is not established.
